The imaging modality least useful for thoracic outlet syndrome is CT scanning. This condition is often driven by soft-tissue and neurovascular compression of the brachial plexus and subclavian vessels, which can be dynamic and position-dependent. MRI of the brachial plexus and cervical spine provides excellent soft-tissue contrast and can reveal scar tissue, muscle hypertrophy, cervical ribs, or other space-occupying lesions that narrow the outlet, as well as direct visualization of the nerves. Ultrasound offers real-time assessment of the neurovascular bundle during provocative arm positions, helping to detect compression of vessels or nerves dynamically and guide diagnosis or management. Nerve conduction studies assess functional impairment of the nerves and can support a neurogenic component when symptoms correlate with compression, even though they don’t show anatomy.

CT scans excel at detailed bone anatomy, which can identify bony causes of outlet compression, but they don’t visualize soft tissues as well as MRI and do not readily capture dynamic neurovascular compression. They also expose the patient to radiation and are less informative for the neurogenic aspects of thoracic outlet syndrome, making CT the least useful option for diagnosing this condition overall.
